This FOCAL International Awards 2009 Submission Form should be used for the following Award category ONLY:

xiii)Award for Archive Restoration or Preservation Project

SUBMISSIONS ENTRY FORM CLOSING DATE FOR ENTRIES 1st DECEMBER 2008 Before completing, please ensure that you have read the RULES of ENTRY. To qualify, the Archive Restoration or Preservation work cited must have been broadcast, screened, launched or published, for the first time, between 1st January 2008– 31st December 2008. The work cited could include any aspect of restoration or preservation of archive moving image content. Supporting visual material is required e.g. restoration work in progress, the complete restored work, plus before and after shots. All Submissions to this category will incur a handling fee of £50 + VAT. Any further Submissions of the same work in any other category will be charged at £300 + VAT. This must be supported by THREE clean identical copies of the production or episode cited on DVD (“PAL - All Zones”). Viewing material should be in English, dubbed or subtitled if possible. If this is not possible, then the orignial language version should be supported by a synopsis in English. Any visual material provided on incorrect formats will not be considered.
